PHP - PDF Conversion

1. Go to http://www.creativityandmagic.com/aspcaseload/startpage.asp. Then click "View All Active" Patients. Then click the patient "Robert James".

2. The two bottom rows of information need to be fixed:

a. The first section is the HTML/PHP Section. Here three different HTML form exists: Treatment Plan, Group Notes and Progress Notes. In these forms the user inputs the information that will be sent to the database. I will send the PHP forms that have already been created to you.

b. The second section is the Dynamic PDF conversion section. Here, the information in the database is retrieved to create the three different PDF documents: Treatment Plan, Group Notes and Progress Notes.

THIS NEEDS TO BE DONE WITH THE FOLLOWING DOCUMENTS:

GROUP NOTES = EXHIBIT B

PROGRESS NOTES = http://66.116.246.6/testing/PROGRESSNOTE.htm

TREATMENT PLANS = EXHIBIT A.

The difficult thing about this is the large BLOG of text pulled from the database must be able to word-wrap from line to line in the PDF document. The ability to limit the number of characters that will fit into a form field will be needed. Also the ability to determine automatic word-wrapping will also be needed. That is why I call it Dynamic PDF conversion.

3. Verify that the HTML to PDF functionality is working properly, this is the heart of the application:

STEP ONE:

The forms accept data from a user, then upon submission, the data is pushed to a database but the form, WITH THE DATA, remains on the screen. Of course the user should be able to edit and update the form as often as they wish.

STEP TWO:

The user then has the option to close that form or PRINT that form.

If the user elects to PRINT that form, the data from the screen is merged into a PDF document. The user can preview the document, then print and or save the document.

FINAL STEP:

THE FINAL PRINTED DOCUMENT MUST BE FORMATTED EXACTLY THE SAME AS THE DOCUMENTS I HAVE INCLUDED IN EXHIBITS A, B & C.

Please contact me for further clarification. The complete application will be database driven. But at this point I want to nail down the code to accomplish the tasks outlined above. After that we will move to the development of the application and the database. And I have most of that done.

WE WILL PROVIDE THE SCRIPT TO CREATE THE COMPLETE MySQL DATABASE.

To choose someone for this job, we will need to a small online demonstration of a PHP form that receives a paragraph of text from a user, then places that text onto a PDF field of limited size. The text must word wrap from line to line.

We want to pay somewhere in the 250-300 range depending on the quality, portability and flexibility of the work.
